Referral and Tenancy Process

At Solace Supported Living, we are committed to providing a seamless and efficient referral process to ensure that individuals receive the support they need as quickly as possible.

The process begins with the receipt of a completed Referral Form, which provides essential information to allow our Tenancy Team to confirm the individual’s eligibility for our housing services. This is followed by a Tenancy Pre-Assessment Session (TPAS), where further details are gathered to ensure a comprehensive understanding of the individual’s needs.

Once all the necessary information has been collected, our Tenancy, Support, and Property Teams collaborate to determine the most appropriate housing solution for the individual. A Home Offer Proposal (HOP) will be made, and if the proposal is accepted, a viewing of the property will be arranged.

After the viewing, and upon final confirmation of acceptance by the individual and their referring agency, the next step is the Tenancy Sign-Up (TSUP). During this session, we will also complete the Support Plan Engagement Confirmation (SPEC), outlining the agreed-upon support plan for the individual moving forward.

Following these stages, the individual will participate in the Property Handover Session (PHOS), where they will receive key information about their new home. Additionally, an Initial Support Session (I-TED) will be held to ensure that the individual’s support needs are clearly identified and addressed. From this point on, the individual will enter our regular support process, receiving ongoing care and assistance tailored to their unique circumstances.

 While the length of the referral process can vary depending on individual circumstances, we strive to make it as swift as possible. When all documentation, including the referral and supporting evidence (such as benefit statements), is provided promptly, the process can be completed in as little as one week.
